New

Agency Monthly Report Form

The new Agency Monthly Report Form is available on line to be downloaded and used to report your monthly statistics to the Food Bank of the Albemarle. The form is "automated" and will streamline a number of the procedures used to submit the monthly report.  Please follow the instructions below to download and use the form and the associated filler/reader.  The form will not be visible to you if you do not download the Filler/reader.  Both download files are small, and the procedure is straight forward.  For each form, following the links below will help you get to the correct location to accomplish the download.

Save the downloaded file wherever you desire.  You will be using that file to install the filler program.

If you have a MacIntosh Computer you will have to follow a different process. Please call the Food Bank at 252-335-4035 for instructions.

Install Formatta Filler 7.0. Once the filler is installed, you can delete the downloaded install program if you desire.  Do not delete the Formatta Filler.  You can use the on-line help file for additional assistance.

After the filler is installed click on this New Agency Monthly Report link and download the monthly report template. Save the New Agency Monthly Report in the same place you saved the Formatta Filler.

The "operation" of the report is straight forward:

  • By pressing the Tab key on your keyboard, you will "visit" each field that can have data entered into it or that will do an action.
  • Pop-up bubbles provide instructions.
  • Dropdown boxes list acceptable inputs where appropriate.  In some cases they will let you enter new data.  The pop-up bubble will provide guidance.
  • Where reporting requires entering a number, appropriate items are totaled.
  • When you reach the end of the report you will be given the opportunity to save, print, and/or send the report.  You should always keep a copy (see instruction 6).  After the monthly report is filled out completely, you only need to save it ONE time.
Note:  A validation check is made on the form before it can be saved and/or emailed.  If designated entries are not made, or the number of individuals served is less than the number of households/families served, the software will alert you that changes must be made.

Save the monthly report file on your computer in the same folder where you saved the Formatta Filler file.  This will make it easy for you to find your Formatta associated files.  Remember to give each month's report file a unique name e.g. Jan08 Agen Rpt to Food Bank.

At the present time you can print and FAX a copy of the report to the Food Bank.  Additionally, by pressing the Send button you will open an email box and email the form to the Food Bank.  By February 2008, our goal is to have every agency that has email capability emailing the report each month. When you email the report, our Agency Relations team will be able to automatically download your data into the proper file at the Food Bank.
